Die medizinische Versorgungssituation von trans* Personen ist von konträren fachlichen und rechtlichen Vorgaben geprägt. Das Verständnis von Trans*Identität hat sich unter Fachpersonen wie in der Gesellschaft in den letzten Jahren essentiell verändert. Mit der 2018 veröffentlichten S3-Leitlinie „Geschlechtsinkongruenz, Geschlechtsdysphorie und Trans-Gesundheit“, dem 2020 verabschiedeten „Gesetz zum Verbot von Konversionstherapien“, das sich ausdrücklich auch auf Geschlechtsidentität bezieht, und dem Streichen der Diagnose „Transsexualität“ aus dem ICD 11 hat die Fachwelt bedeutsame Grundsteine gelegt, um eine verbesserte medizinische Versorgung und psychotherapeutische Begleitung von trans* Menschen zu bewirken.
L. Mahler wird im Vortrag einen Überblick über die bestehende Evidenz zu Entstehungsmodellen der Trans*Geschlechtlichkeit, zur Wirksamkeit von körpermodifizierenden Behandlungen, Möglichkeiten der psychotherapeutischen Begleitung sowie zu psycho-sozialen Belastungen von trans* Personen – insbesondere durch Minderheitenstress – geben.
D. Pauli gibt einen umfassenden Überblick über die wissenschaftliche Evidenz sowie umfangreiche praktische Erfahrungen zur Begleitung von Kindern und Jugendlichen mit Geschlechtsinkongruenz und Geschlechtsdysphorie.
Darauf aufbauend diskutiert M. Günther die wichtigsten Besonderheiten hinsichtlich Behandlungsentscheidungen und Einwilligungsprozessen bei Minderjährigen aus klinischer und ethischer Sicht. Ein besonderer Fokus liegt auf der gesellschaftlichen und fachlichen Kontroverse zur Risiko-Nutzen-Abwägung bei körpermodifizierenden Behandlungen bei Minderjährigen anhand bisheriger Evidenzlage.
K. Teren wird einen Überblick über Transitions- und Re-Transitionsentscheidungen geben. Im Hinblick auf die Erfordernisse einer sicheren und diskriminierungssensiblen Begleitung werden Anforderungen an partizipative Entscheidungs- und Reflexionsprozesse im therapeutischen Setting dargestellt.

Während der letzten Jahre wurde die Begleitung und Behandlung von Kindern und Jugendlichen mit Geschlechtsinkongruenz kontrovers debattiert. Es gibt eine wachsende Zahl von Verlaufsstudien, deren Ergebnisse mehrheitlich positive Verläufe unter Behandlung dokumentieren. Dennoch zeigt sich im kurzfristigen Verlauf ein hohes Mass an Belastungen. Viele trans Jugendliche sind depressiv, suizidal oder leiden an Angststörungen. Die ethische Herausforderung besteht darin, dass ein hoher geschlechtsdysphorischer Leidensdruck einerseits durch medizinische Behandlungen gelindert werden kann, andererseits ein ausreichender Reifegrad für die Einwilligungsfähigkeit und ausreichend Zeit für einen reflektieren Prozess der Identitätsbildung gefordert ist. Eltern sind in die Behandlungsschritte eng einzubeziehen und haben Teil am Entscheidungsprozess. In diesem komplexen Bedingungsgefüge sind individuelle leitliniengerechte Behandlungspfade erforderlich. Die gängige Praxis hinsichtlich diagnostischer, psychotherapeutischer und somatomedizinischer Behandlungsschritte wird im Referat dargestellt und reflektiert.

Minderjährige trans* und nicht-binäre Personen haben bei vorliegender Geschlechtsinkongruenz und einem entsprechenden Leidensdruck Anspruch auf eine Gesundheitsversorgung, zu der auch bedarfsgerechte körpermodifizierende Behandlungen gehören können. Hierfür ist eine individuelle Einwilligung im Rahmen einer informierten Entscheidung notwendig. Wie kann der Prozess der Aufklärung, des Abwägens und Zweifelns so gestaltet werden, dass die minderjährige Person eine für sie stimmige und tragfähige Entscheidung treffen und dass Eltern und Versorger*innen die Entscheidung der minderjährigen Person respektieren und mittragen können?

Wenn wir Personen mit medizinischen Transitionsbedarfen begleiten, stellen sich gleichermaßen Fragen zu Sicherheiten und Unsicherheiten in Entscheidungsprozessen ein. Was, wenn die Transition, doch nicht das Richtige ist? Was, wenn die geschlechtliche Identität aufgrund der Diskriminierungslast nicht gelebt werden kann? Hierbei werden Einflussfaktoren der Entscheidungs- und Reflexionsprozesse dargestellt und hinsichtlich möglicher De-Transitionsentscheidungen reflektiert. Das Thema De-Transition wird einerseits stark diskutiert und instrumentalisiert, andererseits aber auch tabuisiert. Anhand aktueller Studienergebnisse werden Faktoren vorgestellt, die De-Transitionsentscheidungen beeinflussen. Hierbei werden verschiedene Aspekte und Herausforderungen im therapeutischen Kontext sowie in der Gesundheitsversorgung betrachtet. Im Hinblick auf die Erfordernisse einer diskriminierungssensiblen Begleitung werden Anforderungen an partizipative Entscheidungs- und Reflexionsprozesse im therapeutischen Setting abgeleitet.
