Die klinische Forschung zur therapeutischen Wirksamkeit von Psychedelika nimmt in den letzten wenigen Jahren weltweit zu. Der wissenschaftliche Kenntnisstand bei verschiedenen psychiatrischen Indikationen sowie die bereits unter bestimmten Voraussetzungen genehmigungsfähige Anwendung einiger psychedelischer Substanzen in manchen Ländern geben auch in Deutschland Anlass zu Diskussionen über die Möglichkeit eines zukünftigen klinischen Einsatzes. Dabei steht das Feld noch vor grundsätzlichen Herausforderungen inklusive der Notwendigkeit einer kritischen Gestaltung von Rahmenbedingungen für eine mögliche klinische Anwendung. Das Symposium, welches von der Deutschen Gesellschaft für Psychedelische Forschung und Therapie (DGPFT e. V.) gestaltet wird, widmet sich verschiedenen Aspekten der Psychedelika-Thematik.
Im Symposium berichtet zunächst Lea Mertens zum derzeitigen Forschungsstand zu Psychedelika bei Depressionen, dem aktuell hauptsächlich untersuchten Indikationsgebiet. Sie geht sowohl auf abgeschlossene wie auch noch ausstehende Studien ein. Stefan Borgwardt stellt neurobiologische Aspekte der Wirkung von Psychedelika vor mit einem Fokus auf bildgebende Untersuchungen und Konnektivitätsstudien. Hieraus lassen sich mögliche Modelle und ein integratives Verständnis der Psychedelika-Wirkung ableiten. Tomislav Majic widmet sich möglichen postakuten, bzw. anhaltenden Wirkungen nach Einnahme von Psychedelika, wobei sowohl anhaltende therapeutische Effekte nach einmaliger Gabe, wie auch Risiken und Nebenwirkungen angesprochen werden, welche nach Abklingen der akuten Substanzwirkung relevant sein können. Abschließend geht Uwe Herwig auf mögliche Perspektiven eines psychotherapeutischen Einsatzes ein. Dabei werden sowohl andere Indikationsgebiete wie Abhängigkeit und Trauma besprochen, als auch psychotherapeutische Wirkfaktoren und das Spektrum von möglichen Anwendungen hinsichtlich Setting, Frequenz und psychotherapeutischer Einbettung.

Serotonerge oder klassische Psychedelika wie Psilocybin oder Lysergsäurediethylamid (LSD) gehören zu einer Gruppe von psychotropen Substanzen, die akut profounde Veränderungen des Bewusstseins auslösen können und aktuell zur Behandlung verschiedener psychischer Erkrankungen untersucht werden. Die umfangreichsten Erfahrungen und größte Evidenz basierend auf modernen klinischen Studien liegt dabei für Psilocybin in der Behandlung von depressiven Störungen vor. Im Vortrag sollen die Ergebnisse und Behandlungsansätze moderner klinischer Studien mit serotonergen Psychedelika bei Depressionen diskutiert werden. Am Beispiel der aktuell in Deutschland laufenden EPIsoDE Studie (EudraCT Nummer: 2019-003984-24; NCT04670081) zur Prüfung der Sicherheit und Wirksamkeit von Psilocybin bei therapieresistenten Depressionen sollen auch die Besonderheiten und Herausforderungen der Therapie mit Psychedelika herausgestellt werden, sowie ein Ausblick auf notwendige zukünftige Studien gegeben werden.

Postakute Wirkungen von klassischen Psychedelika

Klassische Psychedelika wie LSD, Psilocybin und Ayahuasca unterscheiden sich von den meisten anderen Substanzgruppen durch die zeitliche Dynamik ihrer psychoaktiven Wirkungen. Denn neben den vielfältigen akuten Wirkungen zeigen sie postakute Effekte, die weit über den Zeitraum der akuten Wirkung im ZNS hinausreichen können. Diese Eigenschaften sind es, die maßgeblich für eine mögliche Anwendung von Psychedelika als Therapeutika diskutiert werden. Andererseits stehen Nacheffekte der Substanzen auch in besonderem Maße mit dem Risikoprofil von Psychedelika in Zusammenhang. Während andere psychoaktive Substanzklassen z.T. abhängigkeits-erzeugende oder toxische Eigenschaften haben, spielen postakut zu beobachtende psychische Komplikationen eine besondere Rolle.
Der Vortrag möchte einen Überblick über die Rolle von postakuten Effekten für etwaige therapeutische Anwendungen, aber auch für das Risikomanagement von Psychedelika geben. Dabei werden die für Psychedelika charakteristischen, subakuten „Afterglow“-Effekte, sowie postakute neuropsychologische Konsequenzen dieser Substanzen dargestellt, aber auch postakut zu beobachtende Risiken im Sinne von Psychedelika-bezogenen Störungsbildern.

Der Vortrag bietet nach einer grundsätzlichen Einordnung einen Überblick über die Entwicklung und über mögliche zukünftige Ausrichtungen der Psychedelika-augmentierten Psychotherapie . Zunächst werden historische Wurzeln der Psychedelika-Forschung und klinischen Anwendung geschildert. Daran anknüpfend werden verschiedene Ausrichtungen der Psychotherapie mit Psychedelika beleuchtet und der aktuelle Stand der Forschung kurz dargelegt.
Letztlich wird die Frage aufgeworfen, ob und allenfalls wie Psychedelika in Zukunft in den klinisch-psychiatrischen Alltag eingebunden werden könnten. Dies umfasst insbesondere Indikationsstellung, Setting und Einbettung in eine etablierte Behandlung. In diesem Kontext werden auch besondere Herausforderungen und Risiken benannt und es wird auf aktuelle kontroverse Diskussionen zu dem Thema eingegangen.
