ICD-11 marks several significant changes from ICD-10. For example, there is a refined discussion related to drugs of abuse, lifestyle drinks and medications that cause withdrawal symptoms. Since drug addiction can only be diagnosed if there are symptoms like craving or reduced control over drug intake in addition to tolerance development and withdrawal, careful definitions of harmful drug use and dependence are important when distinguishing between medications that cause neuroadaptations and drugs that cause addiction. With respect to psychotic disorders, ICD-11 follows DSM-5 and de-emphasizes first rank symptoms in schizophrenia. This development has been criticized, because it blurs the line between organic hallucinations and delusions on the one hand and schizophrenia and other psychotic disorders on the other. Misdiagnosis may arise, particularly if there is often no in depth neurobiological examination of all patients.
