Hintergrund:
Die chirurgische Versorgung der Mesenterialischämie bedarf einer hohen klinischen und operativen Expertise. In Anbetracht der guten Langzeitergebnissen der chirurgischen Methoden sollten im Rahmen der interdisziplinären Konferenz die operativen Therapieoptionen erwogen werden. Somit hat eine Revaskularisation der Viszeralarterien in der Ära der endovaskulären Techniken weiterhin einen wichtigen Stellenwert.
Fragestellung:
Das Ziel der Videopräsentation ist die Darstellung einer mesenterialen Revaskularisation mit einem aorto-biviszeralen Bypass und Vergleich der alternativen Anschlussmöglichkeiten.
Material und Methoden:
Das Video dokumentiert eine Implantation eines Bypasses mit dem proximalen Anschluss an der infrarenalen Aorta. Der trunkale Bypass wird retropankreatisch geführt und an die A. hepatica communis angeschlossen. Der mesenteriale Schenkel wird dorsal der V. renalis links geführt und an die A. mesenterica superior distal des Verschlusses angeschlossen.
Ergebnisse:
Unter schonender Präparation der Zielgefäße, Mobilisation der V. renalis links und Einstellung des Op-Situs mit einem Retraktor wird die Operabilität in geübten Händen erreicht. Durch die strukturierte Freilegung der retroperitonealen Strukturen kann man die Verletzung von Milz, Pankreas, Magen und Leber effektiv vermeiden.
Diskussion:
Die retrograde Bypassführung mit dem infrarenalen Bypassanschluss bietet einen sicheren Zugang zu den Zielgefäßen. Optional kann der trunkale Bypass transmesenterial geführt werden, wobei eine chronische Pankreasverletzung riskiert werden kann. Sollten die zirkulären Kalzifikationen den Anschluss an der infrarenalen Anschluss erschweren, bietet sich ein iliakaler Anschluss. Alternativ kann ein subdiaphragmaler Anschluss mit antegrader Bypassführung erwogen werden, wobei hier das wesentliches Risiko der Ösophagus- und Magenverletzung im Vordergrund stehen.
Schlussfolgerung:
Eine Implantation eines aorto-biviszeralen Bypasses mit dem infrarenalen Anschluss ist eine anspruchsvolle Technik, aber mit einem geringen Verletzungsrisiko für die benachbarten Organe.

Background: The use of intravascular ultrasound (IVUS) has been already described as reducing contrast medium usage and radiation exposure during conventional endovascular aneurysm repair (EVAR). Its potential impact in advanced EVAR procedures such as branched EVAR (BEVAR) has not been fully investigated yet. The aim of this prospective study was to test the applicability of this technology in examining the patency and anatomical characteristics of the bridging stentgrafts during BEVAR procedures.
Methods: The routine use of IVUS during BEVAR procedures has been introduced at our Vascular Department in January 2019. In a series of 3 BEVAR procedures a Vision PV . 018 IVUS catheter (Volcano, Philips, Amsterdam, The Netherlands). was introduced distally to the stentgrafts in to the native vessels, IVUS was performed to assess the patency and the anatomy of the bridging stentgrafts. The bridging stentgraft was then examined by retracting the IVUS catheter up in to the aortic stentgraft to verify the adequate deployment, patency of the target vessel, mismatch or kinking of the bridging stentgraft.
Results: An overall number of 11 target vessel were cannulated with the Volcano IVUS system. In two patients a hemodynamic non relevant kink ( < 50% reduction of the diameter) at the level of the renal ostium was found. No relevant stenosis, dissection or kinking of the bridging stentgrafts and target vessel were observed. The completion angiography did not reveal any supplementary information about the stentgrafts patency and anatomy.
Conclusion: IVUS has become an essential tool in assessing patency of the bridging stentgrafts in BEVAR procedures at our Department. It gives a 360 degrees image of the vessel detecting possible stenosed or kinked portions of the graft, and moreover it allows a prompt detection of distal kinking of dissection of the target vessel. Its potentials in reducing the use of contrast media as well as of intraoperative cone beam CT scans should be further investigated.

Neben der klassischen Portkatheterimplantation mittels Freilegung der V. cephalica kann als Zugangsgefäß auch die V. subclavia genutzt werden. Hierbei wird über eine Punktion der V. subclavia der Portkatheter eingebracht und das Portreservoir über eine kleine infraclaviculäre Hautinzision subcutan platziert.
Die Technik bietet neben einer Verkürzung der OP- und Durchleuchtungszeit sowie der besseren Punktierbarkeit des Portreservoirs auch kosmetische Vorteile.
In dem Video zeigen wir die Eingriffsdurchführung und die Ergebnisse von >350 Portimplantationen mittels minimalinvasiver Punktionstechnik.

Excision of a carotid body tumor: Case report and brief video demonstration

Abstract
INTRODUCTION:Carotid body tumors also known as paraganglioma are one of the rare tumors of the head and neck region, that mainly present as slow growing masses combined with dysphagia. Depending on the localisation and the local pressure on the regional nerves, they can present with symptoms such as; dis- and odynophagia, hoarseness, or palpitations. The incidence paraganglioma in Germany (including jugular, tympanic, and aortal paraganglioma) is 0,001%. They are usually defined as benign, rarely malignant tumors with a invasive growth. Surgical treatment still presents the gold standard, with or without preembolization.
CASE REPORT: A 70-year old female patient presented with a painful, slowly growing mass in the left neck region. The patient denied any signs of dysphagia, or neurological symptoms attributable to the tumor. The examination of the ENT was without pathological findings. The ultrasound examination showed a 2,6x1,6 cm hypervascularized mass at the carotid bifurcation (Shamblin II). Indication for surgical treatment was made. Histological examination confirmed the clinical idea of paraganglioma. Additionally a 5 min. video of the procedure, presenting the operative technique of tumor excision without reconstruction of the carotid vessels is presented.
