The effect of protein biosynthesis inhibition on nutrient- and depolarization-induced insulin secretion and oxygen consumption

Background and aims: The nutrient stimulus of insulin secretion, glucose, stimulates protein biosynthesis in addition to its depolarizing effect on beta-cell membrane. To test the relation between biosynthesis and secretion we used the protein biosynthesis inhibitor cycloheximide to modify the effects of glucose in comparison with the purely-depolarizing agent, tolbutamide.
Methods: Freshly isolated and overnight cultured islets were used to simultaneously measure the kinetics of insulin secretion and oxygen consumption rate (OCR).
Results: Freshly isolated and cultured mouse-islets were perifused in 5 mM glucose for 60 min, then 25 mM glucose for 60 min, which led to a continuous increase of the secretion rate until wash-out. When the same protocol was executed in presence of the inhibitor of protein-biosynthesis, cycloheximide, smaller increases of secretion rate and OCR were attained in fresh islets. This inhibitory effect of cycloheximide was clearly less marked in cultured islets. Using 500 µM tolbutamide as stimulus instead of 25 mM glucose, caused moderate increases of insulin secretion and OCR. Again, cycloheximide led to a diminished insulin secretion but the increase of OCR was virtually unchanged. In cultured islets the inhibitory effect of cycloheximide on secretion was less marked than with fresh islets. Here, the OCR was even higher than under control condition.
Conclusion: The protein biosynthesis inhibitor cycloheximide diminished the kinetics of glucose-stimulated insulin secretion and OCR in parallel, whereas it diminished the tolbutamide-induced secretion but not the accompanying OCR increase. Continuous protein synthesis is required for normal insulin secretion, regardless of the type of stimulus.

Knockdown of Etv5 enhances hydrogen peroxide (H2O2) production and impairs viability of pancreatic beta cells

Introduction: The transcription factor ETV5 regulates various biological processes such as cell proliferation and apoptosis, and activates a whole string of obesity- and diabetes-related genes. ETV5 is involved in tissue protection since mice with pancreatic loss of Etv5 show increased severity of pancreatitis and delayed tissue recovery. Here, we investigate the role of Etv5 knockdown (KD) on survival of injured endocrine pancreatic beta cells.
Methods: We generated INS-1E beta cells with doxycycline-inducible lentiviral Etv5 sh-RNA knock-down (KD). Cellular stress was induced by streptozotocin (STZ) and palmitate (PA). We assessed viability (MTS), apoptosis (caspases-3/7 activation), necrosis (assay), ER stress (BiP and CHOP gene expression), oxidative stress (H2O2 production) and gene expression of SOD1, SOD2 and Catalase.
Results: Etv5 KD significantly increases STZ/PA-induced loss of viability without changes in apoptosis but with increased necrosis. Etv5 KD did not change STZ/PA-induced gene expression of ER stress markers BiP and CHOP but significantly increased H2O2 production. STZ/PA induced SOD2 but not SOD1 and catalase. SOD2, which converts superoxide into H2O2, was significantly further increased by Etv5 KD. In contrast, catalase that clears H2O2 into H2O and O2, was not affected.
Summary/Conclusion: Obesity and high glucose promote oxidative stress and loss of Etv5 increases production of both H2O2 and SOD2. Therefore, ETV5 may play an important role in the control of oxidative stress in pancreatic β-cells.

Glucose-induced changes of the ATP/ADP-Ratio in alpha- and beta-cells

Background and aims: The question was whether the increase of glucagon secretion upon glucose lowering is an intrinsic function of the alpha-cell and if so, whether this effect is at the level of the glucose metabolism or electrical events of the plasma membrane.
Methods: Insulin- and glucagon-secretion were simultaneously measured by perifusion of mouse islets and ELISA. To obtain single alpha-cells, islets were incubated with Alloxan before dissociation into single cells. Alpha- and beta-cells were adenovirally transduced with the fluorescent adenine nucleotide indicator PercevalHR. The indicator SNARF was used for pH-correction, the PercevalHR/SNARF ratio was a semiquantitative measure of the ATP/ADP-Ratio.
Results: Upon lowering the glucose concentration from 5 to 1 mM alpha-cells showed a transient increase of the ATP/ADP-Ratio from 4.8 to 5.4. In beta-cells only a monotonous decrease was seen. Raising glucose from 1 to 30 mM resulted in the increase of the ratio-value by ca. 1.1 in alpha-cells. After an initial peak an elevated plateau lasted as long as 30 mM glucose was present. In beta-cells the initial increase by 30 mM glucose had a threefold larger amplitude, followed by a slow further increase. Intriguingly, the transient increase of the ratio-value in alpha-cells upon lowering of the glucose concentration was paralleled by a transient increase of the glucagon secretion in perifused islets. This increase occurred well before the insulin secretion decreased.
Conclusion: The paradox glucagonotropic effect of low glucose may be an intrinsic property of the alpha-cell involving a transient increase of the ATP/ADP-Ratio.

Fragestellung:
Mitochondrien bilden in der Zelle ein tubuläres Netzwerk, welches durch ständige Fusions- und Teilungsprozesse aufrechterhalten wird. Die Balance dieser Mitochondriendynamik ist für die Beta-Zell Funktion essentiell. Der mitochondrial fission factor (Mff) reguliert durch Rekrutierung des Dynamin-Related Protein 1 (Drp1) den mitochondrialen Teilungsprozess. Bei einer gestörten Teilung kommt es nicht nur zu morphologischen Veränderungen, sondern auch zu funktionellen Störungen von Mitochondrien. Beeinflusst wird zudem der Abbau von defekten Mitochondrien, die Mitophagie. Ziel dieser Studie war es, den Einfluss von Mff auf mitochondriale Teilung, Mitophagie und Funktion von Beta-Zellen zu charakterisieren.
Methodik:
Eine verminderte Expression von Mff in glukoseresponsiven MIN6 Zellen und primären Maus Inselzellen wurde mit Hilfe spezifischer siRNA erreicht. Die mitochondriale Morphologie wurde mit Hilfe von MTG Färbung oder einer Tom20 Immunfluoreszenz, das mitochondrialen Membranpotentials nach TMRE Färbung am konfokalen Mikroskop analysiert. Die Insulinsekretion wurde mittels ELISA bestimmt. Die Aktivität der Mitophagie wurde durch spezifische Marker mittels RT-PCR untersucht.
Ergebnisse:
Im Vergleich zu si-kontrolltransfizierten Beta-Zellen mit einer homogenen mitochondrialen Netzwerkstruktur zeigten solche mit einer verminderten Mff Expression einen starken Fragmentierungsgrad. Nach Mff Reduktion waren das mitochondriale Membranpotential und die glukosestimulierte Insulinsekretion im Vergleich zu den si-kontrolltransfizierten Zellen signifikant vermindert. Die Analyse der Mitophagie-assoziierten Gene zeigte mit einer verminderten PINK1 und erhöhten Parkin Expression einen gestörten Abbauprozess der Mitochondrien auf.
Schlussfolgerung:
Die verminderte Expression von Mff in Beta-Zellen führt zu einer verstärkten Fragmentierung von Mitochondrien. Aufgrund unzureichender Mitophagie und der damit verbunden Anreicherung defekter Mitochondrien kommt es zur Dysfunktion von Beta-Zellen, die einen Typ 2 Diabetes mellitus begünstigen kann.

Insulin granule number and mobility as compared with insulin content and secretion after the induction of desensitization or beta-cell rest

Background: The desensitization of insulin secretion by prolonged stimulation may be the initial phase of beta-cell dysfunction in type-2-diabetes. Here, we have investigated the functional consequences of a prolonged exposure to the KATP-channel-blocker tolbutamide and, for comparison, to the inhibitor of insulin secretion, the α2-adrenoceptor agonist clonidine.
Methods: MIN6-cells were transfected with hIns-EGFP and cultured in DMEM-medium. Desensitization or resting state were induced by 18 h exposure to 500 µM tolbutamide or 1 µM clonidine, respectively. Exposed and control cells were perifused with 30 mM glucose or 40 mM KCl in Krebs-Ringer-medium. The number and mobility of secretory granules were analyzed by TIRF-microscopy. MIN6-pseudoislets were perifused utilizing the same stimulation protocol. Insulin secretion and -content were measured by ELISA.
Results: The number of submembrane granules in the cell footprint area was not significantly different between tolbutamide- and control-cultured MIN6-cells, whereas it was significantly increased after clonidine culture. However, both types of pretreatment resulted in similar changes in the parameters of mobility in the z-dimension (granule number per image sequence, short-term granules, granule arrivals). Likewise, the mobility in the x/y-dimension was diminished and the share of the long-term resident granules at the plasma membrane was increased. Perifusion experiments with MIN6-pseudoislets confirmed that both pretreatments diminished the secretory response to glucose and to KCl, even though the tolbutamide culture had decreased and clonidine culture had increased the insulin content.
Conclusion: The diminished granule mobility predicts the secretory response after desensitization or rest better than the granule number or the insulin content.

Fragestellung
Mitochondrien sind durch ihre Stoffwechselaktivität für die Funktion der Beta-Zellen essentiell. In der Zelle bilden sie ein Netzwerk, welches durch ständige Fusions- und Teilungsprozesse regeneriert wird. Hauptmediator der mitochondrialen Teilung ist das Dynamin-related-protein 1 (Drp1), welches durch seine GTPase-Aktivität eine Abschnürung fusionierter Mitochondrien bewirkt. Übermäßige Teilungsprozesse führen zu einer verstärkten Fragmentierung von Mitochondrien. Dieser Prozess wird als Mediator der Pathogenese eines Typ 2 Diabetes mellitus (T2DM) diskutiert. Ziel dieser Studie war es daher, eine Drp1 Funktionseinschränkung in Beta-Zellen zu erreichen und die Auswirkungen auf das mitochondriale Netzwerk zu untersuchen.
Methodik
Eine stabile Funktionseinschränkung des Drp1 in glukoseresponsiven MIN6 Zellen wurde durch Expression der Drp1 Mutante K38A und Selektion mittels G418 erzielt. Die mitochondriale Morphologie wurde nach MTG Färbung und die des mitochondrialen Membranpotentials nach TMRE Färbung am konfokalen Mikroskop bestimmt. Gene der mitochondrialen Dynamik und der Mitophagie wurden durch quantitative Real-Time PCR analysiert.
Ergebnisse
Kontrollzellen zeigten eine homogene mitochondriale Netzwerkstruktur. Demgegenüber führte die Funktionseinschränkung des Drp1 zu einer verstärkten Elongation, teilweise aber auch zur Bildung mitochondrialer Cluster. MIN6 Zellen mit inaktivem Drp1 zeigten im Vergleich zu Kontrollzellen ein signifikant höheres Membranpotential. Mitophagie-assoziierte Gene und solche der mitochondrialen Dynamik wurden darin signifikant höher exprimiert als in Kontrollzellen.
Schlussfolgerungen
Über eine Funktionseinschränkung des Drp1 erreicht man in Beta-Zellen nicht nur eine mitochondriale Elongation, sondern auch eine Steigerung von Mitophagie und mitochondrialer Dynamik. Das erhöhte Membranpotential deutet einen protektiven Effekt auf die Beta-Zellfunktion an. Diese Studie unterstützt das Postulat, die Hemmung des Drp1 als möglichen Ansatz zur Pharmakotherapie des T2DM zu nutzen.

[68Ga]DOTA-Exendin-4 PET/CT zur Detektion von Insulinomen: eine retrospektive Analyse

Fragestellung: Insulinome sind häufige Ursache für rezidivierende hyperinsulinäme Hypoglykämien. Einzig kurative Therapieoption ist die chirurgische Resektion. Eine exakte präoperative Lokalisation und Diskrimination gegenüber fokalen oder diffusen Manifestationen einer Nesidioblastose ist hierfür essentiell. Bisherige bildgebende Verfahren weisen eine unzureichende Detektionsrate auf. In der vorliegenden Arbeit wurde die diagnostische Wertigkeit der [68Ga]DOTA-Exendin-4 PET/CT untersucht. Grundlage ist die Bindung des mit Exendin-4 gekoppelten Radioliganden an den Glucagonlike-peptide-1-Rezeptor, der im Pankreas fast nur in insulinproduzierenden Zellen exprimiert wird.
Methodik: 10 Patienten mit im Hungerversuch klinisch und biochemisch gesichertem Hyperinsulinismus, bei denen die bisherige bildgebende Diagnostik nicht wegweisend oder uneindeutig war, erhielten eine PET/CT eine und zwei Stunden nach Injektion von [68Ga]DOTA-Exendin-4. Die Evaluation der [68Ga]DOTA-Exendin-4 PET/CT erfolgte durch zwei unabhängige Untersucher. Die Intensität der fokalen Traceraufnahme , d.h. der „maximal standardized uptake value“ (SUVmax) und die „tumour-to-background-ratio“ (TBR) wurden zwischen den beiden Zeitpunkten verglichen. Die physiologische Tracerverteilung wurde intrapankreatisch, in der Leber, Milz, Niere, Muskel und Fettgewebe untersucht. Im Falle einer Operation wurden die Ergebnisse des [68Ga]DOTA-Exendin-4 PET/CT mit den histologischen Befunden korreliert.
Ergebnisse: Eine fokale [68Ga]DOTA-Exendin-4 Mehranreicherung wurde in 8/10 Patienten festgestellt. 7 Patienten mit fokaler Mehranreicherung wurden operiert, in allen Fällen konnte histologisch ein Insulinom gesichert werden. 1 Patientin lehnte eine Operation ab. 2/10 Patienten zeigten keine fokale Mehranreicherung, hier wurde die Diagnose einer Nesidioblastose gestellt. Da im zeitlichen Verlauf sowohl der SUVmax als auch das pankreatische Hintergrundsignal anstiegen, resultierte trotz Signifikanz keine bessere TBR.
Schlussfolgerung: Mittels [68Ga]DOTA-Exendin-4 PET/CT können Insulinome, die mit anderen bildgebenden Verfahren nicht entdeckt wurden, zuverlässig detektiert werden.
