Value of blood-based biomarkers for differential diagnosis of Alzheimer’s disease: the ActiGliA cohort

Background: Cerebrospinal fluid and positron emission tomography (PET) biomarkers of Alzheimer’s disease (AD) – amyloid-beta (Aβ) and tau - can identify early disease stages with high accuracy. However, this approach is not suitable for routine clinical care because of the high costs and invasiveness. Implementation of blood-based biomarkers (BBBM) in the diagnostic process may facilitate the identification of persons with AD in the earliest stage possible and may reliably differentiate AD from healthy aging and other non-AD syndroms (NAD) like corticobasal syndrome (CBS).
Method: From the ActiGliA study we included n=27 participants with probable AD, n=17 with subjective cognitive impairment, n=9 with CBS and underlying Aβ pathology (Aβ[+]CBS), and n=26 with Aβ[-]negative CBS. The diagnostic value of BBBM Aβ1–40/1–42, apolipoprotein E4 [ApoE4], glial fibrillary acidic protein [GFAP], neurofilament light chain [NfL], and phosphorylated-tau 181 [pTau181]) in differentiating amyloid-PET-positive (A+) from -negative cases (A-) was examined.
Results: Plasma concentrations of Aβ1-42/1-40 ratio were decreased in A+ cases while ApoE, GFAP, and pTau181 levels were elevated. In the entire cohort, blood and CSF levels of both Aβ1-42/1-40 ratio and pTau181 were significantly positively correlated. ApoE4, GFAP, and particularly pTau181 levels showed a strong negative association with neuropsychological performance. A negative association between pTau levels and brain atrophy was demonstrated. pTau181 levels revealed an area under the curve (AUC) of 0.885 in the discrimination between A+ and A- cases, ApoE4 levels and Aβ1-42/Aβ1-40 ratio showed an AUC of 0.774 resp. 0.815. Regarding differentiation of AD from healthy aging and NAD, elevated NfL levels were associated with a diagnosis of CBS and Aβ[+]CBS, while elevated pTau181 and ApoE4 levels indicated a diagnosis of AD.
Conclusion: BBBMs may be useful in differentiating A+ from A- cases.

Digitale, sprachbasierte Biomarker können demenzbedingte Sprachveränderungen frühzeitig identifizieren und in Kombination mit Chatbots eine ressourcenschonende und niederschwellige Voruntersuchung ermöglichen. Verfügbare Chatbots sind häufig ungeeignet für Menschen mit kognitiven Beeinträchtigungen und führen zu Frustration und Nichtnutzung (Ruggiano et al., 2021).
Die Prospect-AD Studie erhebt und analysiert Sprachdaten von Menschen mit Subjektiver oder Leichter Kognitiver Störung mit Hilfe automatisierter Telefongespräche, die der Chatbot „Mili“ führt. Die Probanden aus den Beobachtungsstudien DELCODE und DESCRIBE des DZNE (aktuell n=135 von geplanten 300 rekrutiert) absolvieren über 15 Monate kognitive Assessments (Wortliste, Verbale Flüssigkeit, Freies Erzählen) in 15-minütigen Telefonaten. Parallel erfolgt eine Nutzerfreundlichkeitstestung (siehe Abb. 1). Kernelemente der Testung sind die System Usability Scale (SUS) sowie semi-strukturierte Interviews (n=19/22 geplanten) zur Nutzerfreundlichkeit von Mili und zur Einstellung zu künstlicher Intelligenz (KI) in der Medizin.
Die SUS ergab eine gute Nutzerfreundlichkeit für Mili (n=62, 8 Missings, Mittelwert 69,8/100, SD 22,1). Die Befragten empfanden Mili als einfach nutzbar (n=44) und leicht zu lernen (n=40). Zehn Probanden fühlte sich bei Nutzung von Mili unsicher, ca. ein 1/3 fand Mili unnötig kompliziert. Die Interviewten waren sehr zufrieden mit Mili und sahen wenig Verbesserungsbedarf, obgleich einige Mili in der SUS schlecht bewertet hatten. KI gegenüber waren die Probanden durchweg aufgeschlossen und sahen sie als integralen Bestandteil moderner Medizin.
Durch enge Kooperation verschiedener Akteure verfügt Mili über eine gute Nutzerfreundlichkeit und war in der Lage, Assessments selbstständig durchzuführen. Es gibt eine Diskrepanz zwischen den Interviewaussagen und den Ergebnissen der SUS. Die positive Einstellung zur KI sollte durch vertrauensvolle Anwendungen in der Medizin verstetigt werden.

Kallikrein-related Peptidase 6 korreliert mit der intrathekalen HSV1-Aktivität bei der Alzheimer-Krankheit

Einleitung:
Das Herpes Simplex Virus Type 1 (HSV1) wird mit der Progression der Alzheimer-Krankheit (AK) in den Zusammenhang gebracht. HSV1 kann durch verschiedene Kinasen die Phosphorylierung in Neuronen triggern. Bei Menschen mit AK konnten wir zeigen, dass die intrathekale HSV1-Aktivität mit der phospho-Tau (pTau) im Liquor positiv korreliert. Ebenso fanden wir, dass die Serinprotease Kallikrein-related-Peptidase 6 (KLK6) bei Menschen mit AK im Vergleich zu gesunden Kontrollen erhöht ist und deutlich mit pTau im Liquor korreliert. KLK6 wird im Gehirn stark von Oligodendrozyten exprimiert. Wir untersuchten, ob KLK6 im Liquor mit der intrathekalen Herpesaktivität und mit den Liquorbiomarkern der AK im Zusammenhang steht.
Methoden:
Bei 104 Menschen mit AK wurde die HSV1-Seropositivität und KLK6 im Liquor bestimmt (ELISA). In HSV1-seropositiven Patienten (HSV+) wurde die intrathekale HSV1-Aktivität mittels Liquor/Serum anti-HSV1/2-IgG Antikörperindices (AI) bestimmt. Per ANOVA wurde KLK6 zwischen HSV1-seronegativen (HSV-, n=24), und HSV+ Menschen mit AK mit niedriger (AI-, n=58) und erhöhter (AI+, n=22) intrathekaler HSV-Aktivität verglichen und AUCs für Gruppenvergleiche errechnet. Die Liquorbiomarker der AK (pTau, total Tau, β-Amyloid(Aβ)42/Aβ40-Quotient) wurden mit KLK6, stratifiziert nach HSV1-Seropositivität, korreliert.
Ergebnis:
KLK6 zeigte sich bei den Menschen mit AK, die eine erhöhte intrathekale HSV1-Aktivität aufwiesen, im Vergleich zu den anderen beiden Gruppen signifikant (p = 0.025) erhöht (AUC HSV+AI+ vs. HSV-: 0,703; HSV+AI+ vs. HSV+AI-: 0,631). KLK6 im Liquor ist in HSV+ Patienten mit allen untersuchten Liquorbiomarkern der AK signifikant korreliert (p < 0.0001), nicht aber in HSV- Patienten.
Schlussfolgerung:
Bei Menschen mit der AK könnte KLK6 im Liquor möglicherweise Einfluss auf die Pathologie der AK haben und sich als Liquorbiomarker für HSV1-vermittelte Toxizität im Gehirn erweisen.

Donanemab in early symptomatic Alzheimer’s disease: efficacy and safety in TRAILBLAZER-ALZ 2, a phase 3 randomized clinical trial

Introduction: In TRAILBLAZER-ALZ donanemab (DONA) cleared brain amyloid plaques, significantly slowing disease progression in early symptomatic Alzheimer’s disease (ESAD).
Methods: TRAILBLAZER-ALZ2 enrolled participants with ESAD and amyloid and tau pathology by positron-emission tomography, randomizing (multicenter) those with low/medium-tau (n=1182) and high-tau (n=552) (missing tau n=2). Participants (randomized double-blind,1:1) received DONA(n=860)/placebo(n=876) IV every 4w for 72w. DONA participants meeting amyloid clearance treatment completion criteria at 24/52w had blinded switched to placebo.
Primary outcomes: Integrated AD Rating Scale(iADRS) change from baseline at 76w in low/medium-tau or combined (low/medium- and high-tau) populations. Statistical testing allocated most power (80%α spend) to low/medium-tau population outcomes, with the remainder for combined population outcomes, including clinical and biomarker assessments.
Results: In the low/medium-tau population iADRS change at 76w:−6.02 (DONA) and −9.27 (placebo) (difference 3.25; 95%CI, 1.88-4.62; P < 0.001), 35.1% slowing of disease progression. Change in Clinical Dementia Rating Scale (CDR)–Sum of Boxes: 1.20(DONA) and 1.88(placebo) (difference −0.67; 95%CI −0.95 to −0.40; P < 0.001), 36.0% slowing. Participants receiving DONA experienced 38.6% less risk of progressing to next disease stage vs placebo over 76w (CDR-Global score, HR=0.61; P < 0.001). Amyloid clearance at 24/52/76w: achieved in 34.2%/71.3%/80.1% DONA-treated participants. Significant, positive results were observed in the combined population. Serious AEs: 17.4%(DONA), 15.8% (placebo), with 3 deaths among DONA patients who experienced serious amyloid-related imaging abnormalities (ARIA). AEs with DONA included ARIA-E (24.0%, 6.1% symptomatic); ARIA-H (31.4%); infusion-related reactions (8.7%).
Conclusion: DONA significantly slowed clinical progression at 76w with a safety profile consistent with earlier studies.

TRAILBLAZER-ALZ 4: directly comparing donanemab to aducanumab on amyloid lowering in early, symptomatic Alzheimer’s disease – results from 12 months

Introduction: TRAILBLAZER-ALZ4 demonstrated superiority of donanemab (DONA) vs aducanumab (ADU) at the 6-month primary endpoint of percentage of participants with early symptomatic AD achieving amyloid plaque clearance ( < 24.1 Centiloids [CL]).
Methods: Participants (n=148) were randomized 1:1 to DONA (700mg IV Q4W [first 3 doses]; 1400mg IV Q4W [subsequent doses]) or ADU. The study is ongoing (duration: 18months).
Results: Baseline demographics and characteristics were well-balanced across treatment arms (DONA N=71, ADU N=69). 27 DONA- and 28 ADU-treated participants had low/medium (intermediate) tau levels based on PET scans. At 12months, 70.3%±6.2% (least-square mean [LSM]±standard error [SE]) DONA-treated vs 21.4%±5.8% ADU-treated participants achieved amyloid plaque clearance (p < 0.001) per florbetapir F18 PET scans. In the low/medium tau subpopulation, 80.0%±9.9% DONA-treated vs 9.6%±5.4% ADU-treated participants achieved amyloid clearance (p < 0.001). The %change (LSM±SE) in brain amyloid levels were DONA:−82.8%±3.1% (baseline [LSM±standard deviation]: 97.59±28.20 CL) and ADU:−56.6%±3.1% (baseline: 102.71±35.30 CL) (p < 0.001). %Change in brain amyloid levels were: DONA−82.7%±4.6% (baseline: 104.97±25.68 CL) and ADU:−57.0%±4.5% (baseline: 102.99±27.87 CL) (p < 0.001). The 6-month brain amyloid reduction with DONA is superior to ADU at 12 months (LSM change±SE, −63.9±2.8 CL; −55.6±3.0 CL, p=0.044). Adverse events occurred in 78.9% of DONA-treated and 82.6% ADU-treated participants. Amyloid-related imaging abnormalities occurred in 29.6% (1.4% serious [n=1]; DONA) and 40.6% (2.9% serious [n=2]; ADU) participants, with higher rates in ApoE4 carriers.
Conclusions: TRAILBLAZER-ALZ 4 provides the first active comparator data on amyloid plaque clearance in patients with early symptomatic AD, demonstrating the higher brain amyloid reduction of DONA vs ADU at 12 months when drug titration has been completed and the treatment regime has stabilized.
